The Trump administration is finalizing two federal rules that would fundamentally reshape how transgender care for minors is funded in America. The first strips Medicaid and CHIP from covering transgender surgeries, puberty blockers, and hormone therapy for anyone under 18. The second goes further: it would block any hospital that provides those services from receiving Medicare or Medicaid funding at all — regardless of whether the specific care was federally funded.

Nearly 45 percent of most hospitals’ revenue comes from Medicare and Medicaid. Pulling that funding isn’t a budget cut. For most institutions, it’s a shutdown threat. Hospitals that want to keep their federal patients would effectively be forced to stop offering this care entirely — not by law, but by financial reality.

Both rules are still moving through the rulemaking process and are not final. A federal judge has already extended an injunction blocking portions of the hospital funding restriction, and legal challenges are expected at every step. But the administration has signaled it intends to push both rules through.

Supporters argue this is a long-overdue protection — that taxpayers should not be funding permanent, life-altering procedures on children who are not legally old enough to vote, drink, or sign a contract. Every major American medical association endorses these treatments as medically necessary. Critics say pulling hospital funding endangers far more than transgender patients, and that these rules amount to a backdoor healthcare ban that bypasses Congress entirely.
